Admissions & referrals
A thoughtful, respectful path to placement
Moving into a residential home is a big decision. We take the time to understand each person's needs and goals, and we work closely with referral partners to make sure Faith Care is the right fit.
Who we serve
Faith Care Residential is designed for adults (18 and older) with qualifying serious and persistent mental health conditions who would benefit from 24-hour support in a small, home-like setting.
Eligibility is determined through the referral and screening process, in coordination with the person's case manager and referral partners. Meeting general criteria does not guarantee placement.
Licensing status: Preparing to open
Faith Care Residential LLC is preparing to open as a Residential Treatment Home. We are not yet licensed by the Oregon Health Authority, and we cannot accept formal referrals or admit residents until a license is issued. You are welcome to contact us now with questions or to stay informed about our opening.
The referral process
Five steps toward the right placement
Every referral is handled with care and confidentiality, at a pace that respects the person.
Availability
We cannot guarantee placement or immediate availability. Openings depend on licensing, current capacity (no more than five residents), and whether our home is a good match for the person's needs.
Step 1: Initial inquiry
A referring professional, family member, or the individual contacts us with general questions. Please do not include health details in your first message — we only need basic contact information.
Step 2: Referral coordination
We work with the person's case manager, the local Community Mental Health Program, or another authorized referral partner. Referral information is shared through secure, appropriate channels — never through this website.
Step 3: Eligibility and needs review
We review the referral information to understand the person's support needs, strengths, preferences, and goals, and whether they meet the requirements for residential treatment home services.
Step 4: Assessment of service suitability
We meet with the person, and with others they invite, to talk openly about whether our home and services are a good match. The person is encouraged to visit, ask questions, and consider their options.
Step 5: Admission planning, when appropriate
If Faith Care is a good fit and a space is available, we plan the move together — including timing, informed consent, personal belongings, and coordination with current providers.
For professionals
Working with referral partners
Residential placements in Oregon often involve the local Community Mental Health Program (CMHP) and other authorized partners, such as hospitals, case management agencies, and other licensed residential programs. In Clackamas County, the CMHP is Clackamas County Behavioral Health.
We welcome conversations with case managers, discharge planners, and other professionals. Formal referral packets should be sent through secure channels arranged directly with us — not through the website form.
